Before using the tool, please gather the following information:

  • Payroll (2019–2023): If unavailable, request it from your Finance department, accountant, or bookkeeper.
  • ZW-flex benefit costs in 2023: Typically found in the WHK assessment, which your Finance team should have on file.
  • WGA benefit costs (if not self-insured for WGA after July 1, 2015): These are benefits that began from 2007 onward and were paid in 2023. You can find them in your UWV WHK assessment.
  • SWGA total costs (if self-insured for WGA since after July 1, 2015): These include all WGA-related benefits paid during your period of self-insurance. Consult your Finance team or social security administrator for support.
